The items checked below are needed to complete your child's registration. Please read and complete the blank forms provided and return them to the school.

o Student Expectations

o Home Language Survey

o Application for Free/Reduce Lunch

o Last School of Attendance Information Form

o District Boundary

OTHER DOCUMENTATION NEEDED FROM PREVIOUS OR CURRENT SCHOOL DISTRICT:

o Discipline History

o Attendance Records

o Withdrawal from school

o CELDT score / report card 07-08

o IEP/504 plan (if they have a plan)

Other documentation needed from parents:

o Copy of Shot Records

o Copy of Birth Certificate

o Proof of address

 

All forms need to be completed and returned to the school’s registrar  in order for the child to be registered.
